The requirements are as follows:
Copy of your U.S. Department of Transportation (DOT) Operating Authority
Must be INTER-STATE/Authorized for Hire/Common Carrier Authority and can include Contract and INTRA-state authority.
Certificate of Insurance
A. Insurance Coverage & Limits
- Liability Insurance (i.e. bodily injury & property damage)
• Minimum Limit $750,000 combined single limit (each accident) - Cargo Insurance
• Minimum Limit $100,00 each occurrence
• If hauling refrigerated cargo, coverage must not exclude coverage for refrigerator breakdown (new title in the middle)Additional Insurance Requirements for Intermodal Freight - Bailee, Hired Auto (vehicle) Physical Damage – must include coverage for container & trailer (chassis)
• Minimum Limit $65,000
B. Named Certificate Holder entitled to notice of cancellation
C. Named Additional Insured
IRS Form W-9 (Request for Taxpayer Identification Number & Certification
• Must be filled out and submitted to Mitchell Bros. for all U.S. Companies
Mitchell Bros. Truck Line, Inc.’s Contract Carriage Agreement
